Good afternoon Prairie Families,
I cannot believe this is the last month of school! As I enter the last month of my first year at Prairie, I find myself reflecting on all that we have done this year. Thank you for all your support and partnership as I learned Prairie traditions and formed great relationships with students, staff, and families. We are ready to finish the year strong!
This week, our students have completed their MAP testing, and for those who were absent, make-up sessions will be held next week.
There will be many fun end of the year events we’re looking forward to this month. We’re also thrilled for the beautiful weather. As the weather changes, our lost and found is once again filling up. Please encourage your child to stop by and check it out if they’ve misplaced anything!
Have a wonderful weekend,
Ms. Lauren Delahanty

*NEW* Health Forms
To comply with Illinois State guidelines, a physical examination – including current immunization records – is required for all children entering an Illinois school for the first time, for children enrolling in kindergarten (even if they previously were enrolled in a District 96 Early Childhood program), and for students entering Grade 6.
REMINDER - Health Forms: Provide required HEALTH FORMS as soon as possible. A physical and immunizations record needs to be on file in the nurse's office by October 15th or the student will be excluded from school. STATE OF ILLINOIS CERTIFICATE OF CHILD HEALTH EXAMINATION FORM. This form must be completed and signed by the pediatrician. Parents must also complete the fields for their child’s name and address, birth date, etc., at the top of the form on pages 1 and 2 in addition to the parent section on page 2, including a parent signature and date.
Other health forms required include VISION, due before Oct. 15, 2024 (Kindergarten and new students) and DENTAL due before May 15, 2024 (2nd and 6th graders).
NEW *Library Materials*
This is a reminder to please start looking under beds, in closets, cars, and backpacks for our loved library books. If you have any books at your house, we would appreciate them being returned before May 17th. We understand that sometimes books have already been returned. We will partner with you to check on our shelves to see if they can be found. Your child may be coming home with a paper list of missing book titles, so take a look in folders to help the search at home easier.
Our final day for book check out is May 10th. All books are due back to the library on May 17th.
NEW *WOW Winners*
Congratulations to this week’s WOW winners!
REPEAT *Did you miss it? April 17: Transition to Middle School*
The "Transition to Middle School" program — designed for District 96 families with fifth-graders who are entering Middle School in the fall took place this past Wednesday. If you were unable to attend, please see the presentation link here.

REPEAT *Newcomer Coffee & Conversation is May 6*
District 96 is hosting a spring Newcomer Coffee & Conversation at 6:30 p.m. Monday, May 6, in the Ivy Hall Mat Room.
For planning purposes, attendees are asked to fill out this form: https://forms.gle/gmJJ6sD3uKsTzMa59. The sign-up deadline is May 3.
This is an event only for parents and guardians.
The Newcomer Coffee & Conversation is an informal opportunity for parents and guardians of newly registered students to ask questions and share their thoughts ~ with other parents and staff ~ about their experiences in the district. These parents might be experiencing the District’s social customs and teaching practices for the first time, just as their students have, and can glean from the experiences of others.
If you missed the last Newcomer Coffee in December, we encourage you to attend this conversation. You are welcome to be a part of this conversation even if you were able to attend the event, so we can hear how you are adjusting to the D96 culture.
Coffee and other refreshments will be served.
Questions? Contact Lubna K. Andrabi, D96’s Community Parent Liaison, @ [email protected] or 847.459.4260,ext. 8136.
