Vaccine Clinics hosted for students 12 and older

Clinics are May 22 and May 26

Kildeer Countryside School District 96 and Doctors Test Centers are teaming up to offer two Vaccine Clinics this month, designed to deliver the vaccine to youth 12 and older.

The COVID-19 Community Vaccination Clinics are to deliver the Pfizer-BioNTech vaccine. Parents and guardians will not have to pay for the vaccine.

The Community Vaccination Clinics are scheduled for:

  • Saturday, May 22 (10 a.m. to 4 p.m.)
  • Wednesday, May 26 (4:30 to 8 p.m.).

The clinics are designed for students who are 12 years old and older.  Youth must be accompanied by a parent or guardian, who must show their photo ID.

Appointments must be scheduled (no walk-ups appointments are available). To schedule an appointment, visit bit.ly/3w5sCLJ. (The appointment for the second vaccine at the Vaccine Clinic at Twin Groves Middle School will be scheduled after the first vaccine is received.)

Please refer to the District’s May 12 email to parents and guardians of Twin Groves and Woodlawn Middle School students for additional information. Once appointments are made, parents/guardians will receive an appointment confirmation email with medical intake paperwork. (You are asked to fill that out prior to arriving for your student’s vaccination.)

Parents/Guardians are asked to arrive at Twin Groves Middle School (2600 N. Buffalo Grove Road, Buffalo Grove) 10 minutes prior to their child’s appointment. If parents/guardians arrive earlier, they are asked to please remain in their vehicle until 10 minutes before their child’s appointment.

 Bring completed authorization documents and insurance card (if you have one).

After youth receive the vaccination, they will need to wait a minimum of 15 minutes for observation.
