Registration in July: Twin Groves (By Appointment)

All District 96 school offices are closed during July. Registration for ALL District 96 schools will be accepted at the Twin Groves Middle School, 2600 N. Buffalo Grove Road, Buffalo Grove, by appointment only on Thursdays in July starting on July 6, 2023.

Please make sure to bring all the necessary documents listed below with you to your scheduled appointment.

REGISTRATION & PROOF OF RESIDENCY

 The following will be needed to prove your residency:

• Copy of your lease/monthly mortgage statement OR proof of real estate closing (such as a Warranty Deed from closing papers)
• Current utility bill 
• Your student's birth certificate, current passport, or visa
• Copy of your driver's license
• ENROLLMENT FORM (complete and return)
• PROOF OF RESIDENCY FORM (complete and return)
• HOME LANGUAGE SURVEY FORM (complete and return)

Also, please keep in mind that you may mail required student health forms to all District 96 school offices throughout the summer. Mail will be received even though school offices are closed.
