SCHEDULE DOCTORS’ APPOINTMENTS TO MEET HEALTH FORM REQUIREMENTS
School offices are closed during July, but you may mail completed forms signed by the physician to the school your student will attend in 2019–20. Mail is received at school offices throughout the summer. FIND SCHOOL ADDRESSES
Health form requirements vary by grade level and for new/returning students….
All students entering Grade 2 must have a DENTAL EXAMINATION FORM on file at their school office by May 15, 2020.
All students entering Grade 6 must have their CERTIFICATE OF CHILD HEALTH EXAMINATION/IMMUNIZATION FORM (signed by the physician) on file in the school office before they will receive teacher assignments/class schedules. This form is due to Twin Groves or Woodlawn Middle School by Fri., Aug.2.
Additional Grade 6 health form requirements include the DENTAL EXAMINATION FORM (due by May 15, 2020).
Playing sports? Will your student entering Grade 6 or 7 or 8 participate in middle school interscholastic sports in the 2019–20 school year? SEE REQUIRED FORMS

BEGINNING BAND & ORCHESTRA REGISTRATION: IT’S NOT TOO LATE!
Just a reminder to register your child for Beginning Band and Orchestra! Students must be going into Grade 5 to join the band and into Grade 4 to join the orchestra. These students will start band and orchestra in August 2019. Please see the links below for more information. We look forward to seeing the students in the fall!

SUMMER READING Grades KDG – 8 have a 2019 summer reading assignment!

SCHOOL AND DISTRICT/BUSINESS OFFICE SUMMER HOURS UPDATE:
District 96 schools are closed throughout July.
In July and through Aug. 2, District/Business Offices (1050 Ivy Hall Ln., Buffalo Grove) will be open to the public 8:00 AM–4:30 PM Monday–Thursday and 7:30 AM–11:30 AM Friday. Offices will be closed July 4 & 5.
Registration for the 2019–20 school year will be taken at the District Office beginning July 8 on Monday–Thursday, 8 a.m.–3:30 p.m. only.
No appointment is needed and students do not need to be present to register.
