SCHEDULE DOCTORS’ APPOINTMENTS TO MEET HEALTH FORM REQUIREMENTS

School offices are closed during July, but you may mail completed forms signed by the physician to the school your student will attend in 2018–19. Mail is received at school offices throughout the summer. FIND SCHOOL ADDRESSES

Requirements vary by grade level and for new/returning students….

All students entering Grade 2 must have a completed and signed DENTAL EXAMINATION FORM on file at their school office by May 15, 2019.

 All students entering Grade 6 must have their CERTIFICATE OF CHILD HEALTH EXAMINATION/IMMUNIZATION FORM (signed by the physician) on file in the school office before they will receive teacher assignments/class schedules. This form is due to Twin Groves or Woodlawn Middle School by Fri., Aug. 3.

Additional Grade 6 health form requirements include the VISION (due by Oct. 15) and DENTAL (due by May 15) exam forms.

Playing sports? Will your student entering Grade 6 or 7 or 8 participate in middle school interscholastic sports in the 2018–19 school year? SEE REQUIRED FORMS

SAVE THE DATE FOR CURRICULUM NIGHT

BEGINNING BAND & ORCHESTRA REGISTRATION: IT’S NOT TOO LATE!

… A reminder to register your child for Beginning Band or Beginning Orchestra! Students must be entering Grade 5 to join the Band and Grade 4 to join the Orchestra. Reading during the summer is critically important for a student’s continued growth. KCSD96 faculty have provided a parent letter, reading resources, and assignment details for all 2018–19 students:

NOTE THIS IMPORTANT INFO CONCERNING STUDENTS’ iPADS

The policy concerning damage to iPads will be changing beginning in the 2018–2019 school year. Please be sure you READ AND UNDERSTAND THESE CHANGES and discuss them with your student. Direct any questions to your student’s school principal.

SCHOOL AND DISTRICT/BUSINESS OFFICE SUMMER HOURS UPDATE:

District 96 schools are closed throughout July. In July and through Aug. 3, District/Business Offices will be open to the public 8:00 AM–4:30 PM Monday–Thursday and 7:30 AM–11:30 AM Friday. Offices will be closed Wed., July 4, in observance of Independence Day.

Note that the District Office building is closed for renovation this summer; District Office staff will be located temporarily in Ivy Hall School, 1072 Ivy Hall Lane, Buffalo Grove (phone extensions will not change). Questions? Call 847.459.4260, ext. 8022.


CONSTRUCTION DETOURS IN DISTRICT 96

Construction detours in Lake County are making accessing some District 96 schools a challenge. This construction and these detours are managed by the Lake County Division of Transportation and the local village–not School District 96. If you have questions or comments regarding access during construction, please direct them to:
