Returning Students
Mail will be received at all District schools throughout the summer. Health forms required for students entering Grade 2 or Grade 6 who were enrolled in a KCSD96 school in the previous school year:
Grade 2 (One Required Form)

Proof of Dental Exam Form OR Dental Waiver Request Form (due by May 15)
Grade 6 (Two Required Forms)
- Certificate of Child Health Examination/Immunization Form, Certificate of Child Health Examination/Immunization Form (SPANISH) OR
Illinois Certificate of Religious Exemption to Required Immunizations and/or Examinations Form (due by October 15) - Proof of Dental Exam Form OR Dental Waiver Request Form (due by May 15)

* For transfer students from out of the state or the country, a health form that is comparable to the Illinois requirements may be acceptable only at the time of first entry into an Illinois school.
Exclusion Policy
In keeping with our Exclusion Policy, students who are not compliant with the physical examination and immunization requirements will not be allowed to attend school. Parents with questions should contact their student's school nurse by calling his or her school.

Tdap Vaccine Requirements
All students entering, transferring, or advancing into 6th, 7th, 8th, 9th, 10th, 11th or 12th grades will be required to show proof of receipt of one dose of Tdap vaccine (containing tetanus, diphtheria, acellular pertussis). Most students may have already received the vaccine and simply need to provide the school with verifying documentation from the family health care provider.
