Middle Sports

Students participating in any middle school interscholastic sport are required to provide the documentation listed below. Students will not be eligible to participate until the required paperwork has been provided and reviewed by the school nurse.

• Student Participation Parent Consent Form OR Formulario de permiso de los padres para que su estudiante participe en actividades athleticas y deportes
• Concussion Info/Parent Sign-off Form OR Informacion Sobre La Conmocion Cerebral

Middle School Interscholastic Sports Requirements Grades 7 or 8 Forms (Two Required): 

• Student Participation Parent Consent Form OR Formulario de permiso de los padres para que su estudiante participe en actividades athleticas y deportes
• Middle School Interscholastic Sports Physical Form 
• Concussion Info/Parent Sign-off Form OR Informacion Sobre La Conmocion Cerebral
