NOTE THIS IMPORTANT INFO CONCERNING STUDENTS’ iPADS
The policy concerning damage to iPads will be changing beginning in the 2018–2019 school year. Please be sure you READ AND UNDERSTAND THESE CHANGES and discuss them with your student. Please direct any questions to your student’s school principal.

 

SCHOOL AND DISTRICT/BUSINESS OFFICE SUMMER HOURS UPDATE:
District 96 school offices will be open weekdays through June 29. School offices will be open by 8 AM. To ensure the administrative assistant will be available at your student’s school at the time you wish to visit, please call the school first to schedule and confirm. SCHOOL PHONE NUMBERS

 

District/Business Office Summer Hours are effective June 4 – Aug. 3. The offices will be open to the public 8:00 AM–4:30 PM Monday–Thursday and 7:30 AM–11:30 AM Friday. Offices will be closed Wed., July 4, in observance of Independence Day. Note that the District Office building will be closed for renovation this summer; District Office staff will be located temporarily in Ivy Hall School, 1072 Ivy Hall Lane, Buffalo Grove (phone extensions will not change). REQUIRED 2018–2019 HEALTH FORMS DUE TO SCHOOL OFFICE BY AUG. 3
Health form requirements vary by grade level. MORE INFO
All students entering Grade 6 have health form requirements that must be met before they will receive their schedules and their locker assignments. The CERTIFICATE OF CHILD HEALTH EXAMINATION/IMMUNIZATION FORM  (Spanish)(signed by the physician) is due to Twin Groves or Woodlawn Middle School by Fri., Aug. 3.  Please deliver or mail completed forms to the office of the school your student will attend in the 2018–19 school year. DOES YOUR 8TH-GRADER HAVE A POSITIVE LUNCH ACCOUNT BALANCE?
Positive lunch account balances (any amount) may be transferred to a younger sibling enrolled in District 96, Grades 1–8, for the 2018–19 school year.
Make or request your transfer NO LATER THAN Fri., June 8.
To make the transfer online:

Log into your K–12 PAYMENT CENTER account and select “Transfer Funds” on the left side of the website home page. OR
To request a transfer if you have not yet established an account:

Send a “Transfer Request” email to Michelle Mize in the Business Office. Include the folllowing info: Names of BOTH the graduating 8th grader AND the remaining D96 sibling whose account should receive the transferred amount. Note: All un-transferred positive lunch account balances in excess of $10 will automatically be refunded. A check will be issued on June 20 and will be mailed to the home address included in our database.

SCHOOL SUPPLY KITS FOR 2018–19
The last day to order a 2018–19 School Supply Kit (optional) from the PTO is Fri., June 1. Kits will not be available for purchase after June 1. Place your order online on the PTO Membership Toolkit for the school your student will attend in 2018–19. If your student is new to that school, you will need to create an account.
